TLC  is a privately owned home health care staffing agency that is based in Crestline, Ohio.  TLC services counties in Northern and Central Ohio, and provides home health care services, as well as nurses aide training classes. We have received accreditation by the Community Health Accreditation Program (C.H.A.P.) which allows us to provide Skilled,  Passport, Waiver, Private Insurance, and Private pay home care cases.

The staff of TLC Home Health Care, Inc. will to the best of their ability, provide quality health care, which promotes the dignity and independence of patients and their families. Clients will be admitted for service regardless of their age, race, religion, sex, handicap, diagnosis, national origin, ancestry, color, or Vietnam-era Veteran status.

The health care team is made up of Registered Nurses, Licensed Practical Nurses, and Paraprofessional staff. Together, they use a systematic approach to the process of individualized care planning and delivery. Medical necessity determines the type and frequency of the service delivered. The health care team focuses on providing comprehensive, coordinated services necessary to achieve optimal functioning in the home environment.

Clients and appropriate caregivers are kept informed on condition changes and all procedures are explained prior to performing. Refusal of care will be noted and respected. Skilled professionals and paraprofessional staff are encouraged to insure their own mental and physical health in order to recognize and act upon patient needs. Further, the staff is supported in their pursuit of education and networking to assure competency and capability.

Educational programs are given on a monthly basis, keeping our staff competent and informed of new changes in the health care field.
